Home Health Services

Home Health services are for those individuals requiring short term skilled interventions of either a nurse or physical therapist. Nursing services cover: medication management, wound care, catheter care, patient teaching, and infusion services. Physical therapy services cover: patients with mobility, balance, and deconditioning issues; in addition to post-op joint replacement services. Referrals are made through either a physician or hospital discharge planner.

Hospice Service

Hospice services are provided for those individuals who have been diagnosed with a life limiting illness or injury and have chosen to seek comfort care verses curative options. Hospice is provided through an interdisciplinary team of physicians, nurses, social workers, chaplains, and aides. The patient/family guides the plan of care in conjunction with the physician and the nurse so that their decisions/choices are the guiding principles of the hospice care delivery. Referrals can be made by anyone, however the order for hospice must come from a physician.
