Director of Case Management
The Director of Case Management reports to the Chief Financial Officer and oversees all employees within the Case Management department. The Director of Case Management will be responsible for Interviewing, hiring, evaluating and retaining case management staff who demonstrate competencies consistent with RMCHCS’ mission, vision and values. They will work with physicians and multidisciplinary team members to develop a plan of care for each patient from admissions through discharge that facilitates improved patient throughput, flow, outcomes and length of stay. The Director of Case Management will also monitor compliance with DNV-GL and CMS Conditions of Participation, regulatory agency standards and industry best practices. The Director of Case Management is responsible for completing all duties as assigned.

Education:
•Graduation from an accredited school of nursing is required.
•Bachelor’s Degree in Nursing or healthcare-related field is preferred.

Experience:
•Minimum of three (3) years’ clinical nursing experience required.
•Two (2) years utilization management/case management preferred.

Required Licensure/Certification(s):
•Current License to practice Professional Nursing in the State of New Mexico or other health related field required.
•Current certification in utilization review/quality or case management is required.
•Current certification in Basic Life Support (BLS) is required.

Essential Job Competencies/Standards of Performance
•Provide education and training, coaching and development, role modeling and mentoring for all assigned staff to ensure cohesive teamwork and competency.
•Work with physicians and multidisciplinary team members to develop a plan of care for each patient from admissions through discharge that facilitates improved patient throughput, flow, outcomes and length of stay.
•Ensure patient is progressing towards desired outcomes by continuously monitoring patient care through assessments and/or evaluation and assures that documentation supports CC/MCC capture, POA, and CMS/TJC quality measures.
•Monitor compliance with CMS Conditions of Participation, regulatory agency standards and industry best practices.
•Practice in preparedness for Recovery Audit Contractors and Pay for Performance requirements.
•Identify and resolve barriers that hinder efficient patient care.
•Maintains all required licenses and certifications. Submits updated verifications to Human Resources (HR) upon receipt of updated documentation.
•Actively upholds RMCHCS’ Mission, Vision and Core Values. Follows all policies and procedures.
