Registered Nurse - Home Health & Hospice
Position Summary:
Under the general direction of the Director of Home Health, the Registered Nurse (RN) is responsible for provision of age-appropriate and culturally sensitive nursing care to patients of all ages, in their own homes. Formulates an appropriate nursing plan of care, with goals mutually set by the RN and the patient and/or caregiver. Formulates an appropriate and safe aide plan of care. Supervises the Home Health Aide every 14 days in the patients’ homes. Assesses the effectiveness of interventions and education to the patient and their family.
Basic Qualifications:
Education:
•Associate's Degree in Nursing or Diploma Program required.
•BSN preferred.
Experience:
•At least one (1) year previous clinical experience in area of expertise.
•Ability to read, communicate and document effectively through oral, written, or electronic means.
•Previous computer experience and excellent customer service skills.
•Ability to prioritize and respond promptly to a changing workload.
Licensure & Certifications:
•Current LPN license to practice Nursing in the State of New Mexico or acceptable compact state licensure.
•Must complete and maintain ACLS certification within three (3) months of hire.
•Must complete and maintain PALS and TNCC within 6 months of hire.
•Current verification in Basic Life Support. (If BLS verification is not current upon hire must be obtained within three (3) months of hire.)
•CPI Certification and/or experience in IV Therapy preferred.
Essential Job Responsibilities:
•Assess patient and develop plan of care; set goals mutually with the patient and/or caregiver. Assess all factors that affect patient’s status, including psychosocial, emotional, environmental, and financial circumstances.
•Obtain referrals for other skilled services as indicated.
•Provide emotional support and education to patients and their families.
•Develop an appropriate and safe aide care plan and supervise the home health aide at least every 14 days in the patients’ homes.
•Accurately document assessment, care plan, OASIS, goals, medications, communications, and interventions.
•Review and revise the plan of care every 60 days (90 days for the first two hospice episodes) and as needed.
•Promptly report significant changes to the physician.
Standards of Performance:
•Acquire all education as required, including continuing nursing education hours and mandatory in-services; maintain professional licensure.
•Independently administer intermittent nursing care and therapeutic treatment in the patient’s home as ordered by the physician, in a safe and timely manner, utilizing nursing skills to provide safe and effective care.
•Evaluate and document the effectiveness of the plan of care as developed by all members of the interdisciplinary team working with the patient.
•Actively participate in hospice interdisciplinary group (IDG) meetings and Home Health Case Conferences.
•Promptly submit accurate and complete documentation.
•Supportive of the Compliance Program to include the Code of Ethics and Corporate Compliance.
•Sets an example to all staff in their daily activities.
•Assists in orienting and training new employees, Nurse Interns/new graduate nurses and vocational nurses, student nurses and contract personnel. Serves as a resource person for departmental employees in resolving patient care problems.
•May serve as Charge Nurse for shift as designated. Insures completion of patient care and functioning of unit, directs Nursing staff, facilitates movement of patients from one level of care to another. Additional Information Position Type : Pool/PRN/Per Diem Shift : Day Contact Information
