Case Management


Dealing with an illness can be a very difficult time for patients and their loved ones. At RMCHCS, experienced and sensitive Case Manager Nurses and Social Workers are available to assist patients and their families facing an illness, hospitalization and medical treatment.

Our case management team acts as patient advocates by working with patients and families to discuss available health care options when the patient is medically ready for discharge. Options may include home health care services, skilled nursing, acute rehabilitation services or hospice care based on patient preference, medical necessity, qualifying criteria and the patient's payer requirements. The Case Management staff can discuss other resources that may be available in the patient's community that could assist with overall wellness when the patient is medically ready for discharge.
